Why we collect your information
The University collects the information provided in this form to assess and manage your organisation's registration as a student placement host. This includes verifying your organisation's suitability, coordinating placements, and communicating with you throughout the placement process.
What we collect
We may collect the following information about your organisation and its representatives:
- Organisation name, address, and contact details
- Details of the nominated placement supervisor
- Information about your organisation's activities and placement capacity
- Workplace health and safety information relevant to hosting students
How we use your information
Your information may be used to:
- Assess and verify your registration as an approved host organisation
- Match and allocate students to appropriate placements
- Share relevant details with students and faculty staff involved in managing the placement
- Meet the university's legal and reporting obligations

- Relevant faculty staff and placement coordinators
- Students allocated to your organisation for placement
- Third parties where required by law

Your information is stored securely within PIPS and is only accessible to authorised university staff.

Privacy Collection Notice — Students
The information within this form is being collected by the University to assess and manage your participation in the university's student placement program. You can contact our placement team.
The information you provide will be used to assess your placement submission and its suitability, to complete any legal agreements required for your placement, and will be retained for reporting and compliance purposes. Your information will only be accessed by authorised staff for the purpose for which it was collected, and will be protected by appropriate technical and organisational measures against accidental or unlawful destruction, misuse, loss, alteration, or unauthorised access or disclosure.
If your placement is approved, relevant information will be shared with your host organisation in accordance with the legal agreement for your placement. Your information may also be shared with relevant accreditation bodies to meet reporting requirements associated with your degree program. If you do not provide all the information requested, we may not be able to assess or approve your placement.
Your personal information will only be retained for as long as required for the purpose it was collected, and in accordance with our legislative obligations and the University's retention and disposal authority. Your information will not be disclosed to any other party unless you have given consent, or we are authorised or required to do so by law.
You may request access to or correction of information we hold about you, or exercise other applicable privacy rights, by contacting our placement team.
University of Melbourne collects and manages your personal information in accordance with the University's Privacy Policy, the Privacy and Data Protection Act 2014 (Vic), the Health Records Act 2001 (Vic), the Privacy Act 1988 (Cth), and where applicable, the EU General Data Protection Regulation 2016/679 (GDPR).
